Doesn't happen very often, here in Queensland, Australia. The majority of low bridges are fitted with "tell-tails" and video recording. The penalty for hitting one (especially rail bridges) is now in excess $AU 5,000. Last major collision here in Qld was not the fault of the driver, the height was posted at 4.3.metres. The road authority has just repaved the road, adding 200mm to the surface. Just a bit hard for a 4.2 m high vehicle to pass under a 4.1m opening. Road authority also, very uncommonly, paid for the repair and the damaged freight.
